Free Printable Mother's Day Questionnaire + Gift Card Holder

Give mom a gift card to her favorite place, wrapped in a sweet card and questionnaire for kids to fill out!

Looking for a special gift from the kids for Mother's Day? My favorite Mother's Day gifts from my daughter were the questionnaires she'd fill out (with the help of my husband) that had the hilariously adorable answers. This Free Printable Mother's Day Card not only holds a gift card just for mom, but it also includes a questionnaire! I think moms, grandmas, and aunts everywhere are going to love receiving one of these special cards this Mother's Day!

Download and print the Mother's Day card, backer, and envelope. For best results, I recommend printing them all on cardstock. Use a paper trimmer or scissors to cut it out following the outer guidelines.

Next, line a ruler up next to the two horizontal fold lines, and press a scoring tool or bone folder next to the ruler and run it down the fold line a couple of times. Fold the card accordion-style, so that the front of the card is stacked on top of the bottom section of the card.

Unfold the card, and have the kids (or helpful adult) use a pen, colored pencils, crayons, or markers for the kids to fill in the answers about their mom, grandma, aunt, or other motherly figure!

As a mom myself, the questionnaire with answers from kids is my favorite part of the gift.

Apply your favorite paper adhesive to the back of the bottom section of the card. Center it on top of the backer piece and press it down well.

Add a gift card to mom's favorite store or restaurant over the rectangle on the card and hold it in place with a glue dot. Don't forget to have the kids sign their name at the bottom on the "From" line!

Set the card aside to assemble the envelope. First, carefully cut the envelope out with scissors following the outer guidelines. Next, run the scoring tool over the lines inside the envelope to help it fold with ease.

Fold all of the tabs to the back of the envelope. Lay the envelope face-down with the triangle tab at the top. Fold the bottom tab up, and add some paper adhesive to both side edges. Then, bring the side tabs over, and place them down on top of the adhesive on the bottom tab. Hold them in place until the adhesive sets.

Fold the card down, accordion style and press it flat. Slide it into the envelope, and the card is ready for gifting! You can use adhesive or a sweet sticker to keep the envelope closed, and a message can be written to mom on the front.
